For the purposes of the appointment of PDP co-chairs, prospective candidates (nominees) and eligible resource members are required to use the following two template nomination letters when submitting their candidatures: one to be completed by the first nominating organisation and the other by the second nominating organisation. This is also applicable to self nominations.

In both instances, the nominator (whether the proposer or the seconder) must be a duly authorised officer of the respective organisation.
